The exact number of implants used depends on the situation and varies from patient to patient, which is why the treatment can sometimes be referred to as “All-on-X” rather than a specific number such as All-on-4 or All-on-6.

Unlike traditional dentures, which rest on the gums and can move during eating or speaking, All-on-X teeth are securely attached to dental implants and remain fixed in place.

What Is the Timeline for Treatment?

Thanks to modern advances in dental implant science, many patients are able to receive a fixed provisional set of teeth on the same day or shortly after implant placement. These provisional teeth are custom made and will look similar to the final result, allowing you to eat, speak, smile, and function while your implants heal and integrate with the surrounding bone.

During this healing phase, you will essentially be “test driving” your new smile and bite. This period allows us to evaluate function, comfort, speech, and esthetics before fabricating your final restoration.

After approximately 3–6 months, depending on healing and individual circumstances, your final prosthesis is fabricated. At Palm Beach Periodontics, this is typically constructed from milled zirconia reinforced by a titanium framework, providing an excellent combination of strength, esthetics, cleanability, and long-term durability.

Who Is Typically a Good Candidate?

Many of the patients who benefit most from All-on-X fall into one of several categories.

If you have numerous teeth with advanced decay, severe periodontal disease and bone loss, recurrent infections, or extensive dental breakdown, restoring each tooth individually may not always be the most predictable or esthetic long-term solution.

In some situations, replacing the remaining teeth with a full-arch implant restoration may provide a more stable, predictable, and esthetic outcome.

Patients Who Wear Dentures

Patients who struggle with loose dentures often seek a solution that allows them to eat, speak, and smile with greater confidence.

All-on-X can provide significantly improved stability and function compared to traditional removable dentures.

Patients Facing Extensive Dental Treatment

Occasionally, patients require extensive treatment on nearly every remaining tooth. While saving natural teeth is almost always our preferred approach when feasible, there are situations where the long-term prognosis of the dentition is poor.

Many of these patients have reached a point where continuing to repair individual teeth becomes increasingly costly, complex, and unpredictable.

For these patients, All-on-X may offer a more predictable path forward.

Should I Remove All My Teeth to Get All-on-X?

One of the most important parts of our evaluation process is determining whether your natural teeth can be predictably maintained.

While All-on-X can be an excellent treatment option, preserving healthy natural teeth is almost always preferable when possible. Our goal is not to recommend implants at all costs, but rather to help you choose the treatment option that offers the best long-term prognosis, function, and quality of life, while also recognizing that each patient has unique goals and priorities.

In some cases, patients strongly prefer to preserve their natural teeth whenever possible, even if doing so may require additional treatment, financial investment, and compromise in the final esthetic outcome. In others, patients would prefer not to continue investing in a severely compromised dentition and instead pursue a more definitive full mouth implant solution.

Every patient is different, and our role is to provide you with the information, expertise, and guidance necessary to make an educated and informed decision about your dental treatment.

How Much Bone Is Needed?

One of the biggest misconceptions about dental implants is that a patient must have a large amount of bone available.

While adequate bone is important, modern implant planning techniques allow many patients with moderate to severe bone loss to qualify for treatment.

Using three-dimensional CBCT imaging, we can evaluate the quantity and quality of available bone and determine whether implants can be placed immediately or whether additional procedures may be recommended.

One of the major advantages of the All-on-X concept is that implants can often be strategically positioned to maximize available bone and reduce the need for extensive grafting procedures.

In some cases, bone grafting or sinus augmentation procedures may still be recommended. In others, they may not be necessary. Every patient presents with unique anatomy and treatment considerations.

Am I Too Old for All-on-X?

One concern we frequently hear is whether a patient is “too old” for dental implants.

In reality, age alone is rarely the determining factor. Overall health, healing capacity, and treatment goals are generally far more important than a patient’s chronological age.

We routinely see patients in their 70s and 80s successfully undergo implant treatment and enjoy significant improvements in comfort, function, and quality of life.

Does the Procedure Hurt?

Fear of discomfort is one of the most common concerns patients have when considering All-on-X treatment.

While All-on-X is a significant surgical procedure, thanks to modern surgical techniques, sedation options, long-lasting local anesthesia, and advances in implant dentistry, most patients report significantly less discomfort than they anticipated.

At Palm Beach Periodontics, our patients receive a specialized pre- and post-surgical care kit that includes products designed to improve comfort and support healing. For patients who desire additional comfort during recovery, we also offer long-acting local anesthetic options that can provide numbness for up to several days following surgery. For patients with dental anxiety, oral and IV sedation options are available.

While soreness, swelling, and temporary dietary restrictions are expected during the initial healing period, many patients are pleasantly surprised by how manageable recovery is. Most patients are able to return to normal daily activities quickly.

Our team will provide detailed instructions, medications when appropriate, and close follow-up care to help ensure a smooth recovery experience.

What Maintenance Is Required After Treatment?

One of the most common misconceptions about All-on-X is that once treatment is completed, no additional maintenance is necessary.

While the prosthesis itself cannot develop cavities, the surrounding tissues and supporting implants require diligence in maintaining cleanliness.

Without proper maintenance, dental implants can develop inflammation, bone loss, and infection, a condition known as peri-implantitis.

For this reason, we will provide you with the knowledge and tools necessary to maintain your new prosthesis, as well as schedule regular professional maintenance visits and examinations. Depending on your specific restoration and risk factors, this may include routine hygiene visits as well as annual removal of the prosthesis at our office for a more comprehensive examination and cleaning.

This allows us to evaluate the implants, surrounding bone levels, prosthesis, screws, and soft tissues in areas that cannot be fully visualized while the prosthesis remains in place.
